Abstract: In retrospective reanalysis, an explanation- first LLM applied to routine HPO terms and variant tables produced clinically relevant gains in diagnostic yield, surfaced overlooked pathogenic findings, and generated biologically grounded hypotheses. These results motivate prospective multicenter evaluation with predefined end points, calibration reporting, and comparator baselines.

Abstract: Individual pediatric cancers are rare diseases. Data resources that aggregate samples from multiple institutions are a force multiplier. Using the Single-cell Pediatric Cancer Atlas as the primary example, I'll describe how the Alex's Lemonade Stand Foundation Childhood Cancer Data Lab builds with reuse as a design goal and the science it yields. Lessons around processing, curation, and reuse will be broadly applicable to rare disease communities trying to build data resources and infrastructure.
